  1. Looking at the final product - did the team deliver what you agreed upon? Please choose:
    • Yes, everything is there, and it exceeds my expectations!
    • Yes, the final version of the product is just what we agreed upon
    • Yes, to some extent, the created application is more or less what we agreed upon
    • Not really, the created application does not meet my requirements
    • Not at all, created application needs to be redone
    • Cannot say, team has not shown to me the final version or has not made the application accessible for me
  2. Are you satisfied with the quality of the final product? Please choose:
    • Yes, the product is flawless. There are no bugs or only some very minor annoyances.
    • Yes, to some extent. The bugs found are known to the team and they are minor issues.
    • Not really, but I do believe that given a one more month the team would be able to fix the application
    • Not at all, the quality is below any reasonable expectations [please add more details in this case]
    • Cannot say, the team has not show to me the final version of the product or has not made the product accessible for me
  3. What are your plans with the final solution?
    • The solution is already used by actual users.
    • I plan to make the application available for end users in the next few months.
    • The application requires some rework, after which I believe it is possible to make it available for end users.
    • The application itself is not usable by end users, but it is good as a preliminary prototype.
    • The application has little or no value whatsoever [please give some details in this case]
  4. Would you consider ordering software from the same team again?
    • Yes, of course!
    • Yes, if the team gets rid of some of the annoyances we faced, I would consider ordering software from them again.
    • Not really. The team needs to make serious changes in either how they work or how they ship quality products, but as of now I most likely would not order from them again.
    • No, never!
